NEW
PATIENTS

To establish with our practice, please call the office in which you would like to be seen. Our New Patient Coordinator will gather your information and schedule you for a New Patient appointment. A new patient packet will be mailed to you, or you can download the packet from the 'Forms' section of this website. The questionnaire is lengthy and detailed. You will want to complete it before the day of your appointment. 

 

To help your visit run as smoothly as possible, please come prepared. Be sure to have a photo ID, your insurance card(s), copayment, and the fully completed New Patient Health Questionnaire. These will be collected at check-in.

You will be instructed to arrive 30 minutes prior to the scheduled appointment time. This allows for time to complete the full registration process and for the nurse to create your chart and input all the information from your health questionnaire without cutting into the provider’s time with you.   

 

Please keep in mind, this appointment is to get you established with your new provider. It will not be billed to insurance as an annual wellness or preventive visit. The provider will order screening labs and preventive care for a subsequent visit.
